I was having the same issue using the LM3150 and two FDD8647L FETs. I am getting 2.16v instead of the expected 5v. My design is all ready got boards printed and and found out the voltage was wrong after building two dc to dc converters. Turns out the footprint I was using had the pins labeled wrong, reversing my source and gate. Looking for a solution that has the same values (40v/14A) but has the gate and source pins reversed(the gate is pin two and the source is pin one). Im surprised im getting any output at all tbh.
