I have further information. All the failures happened after two components were swapped for others. The input bridge rectifier caps (swapped from some no name brand to Panasonics) and the inductor (swapped from Bourns to a no name brand). The input caps are not going to be the problem. This leaves the inductor. I'm trying to get the datasheet for whatever part they put on there. I suspect a low inductance value to begin with, and when the inductor began to saturate, the inductance fell further and added to the problem.
