Since CHG is 11.8V, your charge FET should be on, however the CHG output is controlled relative to CPCKN which should be approximately the same as PACK-.  It would be good to measure CHG with respect to PACK- to determine if the charge FET is being commanded on.

Since DSG is low, the IC apparently thinks the cells are under voltage.  With CHGST pulled high, it will turn on CHG in under voltage, but other faults should keep CHG off. Check that the cell count configuration in EEPROM is set to the cells you have connected.   With DSG low, if the battery voltage is about the same as the charger voltage, that is less than the discharge FET body diode drop, charge current will not flow.  Measuring the voltage between battery- (VSS) (+) and PACK- (-) may be helpful.  If this is more than about 0.7V, check that the FETs are installed correctly and that the CHG and DSG voltages are reaching the FET gates.  Also check for damaged FETs.
